What is actually producing the symptoms
The sciatic nerve is the longest and widest nerve in the body, running from the lumbar spine through the gluteal region and down the leg. Symptoms in the distribution of this nerve can arise from compression or irritation at several distinct points along its course, at the nerve root within the spine, from a disc prolapse or degenerative change; within the piriformis or deep hip rotators, in what is known as piriformis syndrome; or at other points along the peripheral nerve. Each of these presentations has a different clinical profile, responds to different treatment approaches, and carries different implications for the pace and nature of recovery.
A patient who has been treated for lumbar nerve root compression when the true source of their symptoms is piriformis syndrome will not improve with the treatment they are receiving, however well it is delivered. This is not an uncommon scenario, and it is one of the most frequent reasons patients present to Swiss Touch having been through a course of physiotherapy elsewhere without meaningful improvement.
